Spring SEASON 2023

Contact, Release Forms, and Registration


All three steps outlined below must be completed in order to register. If you have any problems with any step, please email coordinator@greenvillejtt.com.
    Step One:
    Please complete this form for UJTT Communications.

    Step Two:

    Read Release Form, Parent Code of Conduct and All Risk Waiver Statements and Sign for Acceptance.
    RELEASE FORM
    I, the undersigned, agree to be legally bound, hereby, for myself, my heirs, executors and administrator, waive and release any and all right and claims for damages I may have against the USTA, UPTA and GJTT, the host facility, and any and all sponsors and their representatives, successor, and assigns for any and all injuries suffered by me in said event. I have read and understand the foregoing releases, waivers and indemnity agreement.

    Medical Release: I hereby consent to emergency first and other medical procedures, or hospital service that may be rendered by or at accredited hospitals, by appointed physicians, which at the time of injury or illness seem reasonably advisable. I further understand that I will be responsible for payment of any such medical procedures.


    Media Release: I hereby give USTA South Carolina, UPTA, GJTT and their representatives the irrevocable right to use my name, picture, photograph, or other likeness in all forms and media and in all manners. This includes but is not limited to print and the web.  I waive the right to inspect or approve the finished version(s), including any written copy that may accompany it.

    PARENT CODE OF CONDUCT

    The essential elements of character building and ethics in sports are embodied in the concept of sportsmanship and six core principles:  trustworthiness, respect, responsibility, fairness, caring, and good citizenship. The highest potential of sports is achieved when competition reflects these "six pillars of character."

    I therefore agree:
    1. I will not force my child to participate in sports.
    2. I will remember that children participate to have fun and that the game is for youth, not adults.
    3. I will inform the coach of any physical disability or ailment that may affect the safety of my child or the safety of others.
    4. I will learn the rules of the game and the policies of the league.
    5. I (and my guests) will be a positive role model for my child and encourage sportsmanship by showing respect and courtesy, and by demonstrating positive support for all players, coaches, officials and spectators at every game, practice or other sporting event.
    6. I (and my guests) will not engage in any kind of unsportsmanlike conduct with any official, coach, player, parent or spectator such as booing and taunting; refusing to shake hands; or using profane language or gestures; or physically abusing anyone.
    7. I will not encourage any behaviors or practices that would endanger the health and well being of the athletes.
    8. I will teach my child to play by the rules and to resolve conflicts without resorting to hostility or violence.
    9. I will demand that my child treat other players, coaches, officials and spectators with respect regardless of race, creed, color, sex or ability.
    10. I will teach my child that doing one's best is more important than winning, so that my child will never feel defeated by the outcome of a game or his/her performance.
    11. I will praise my child for competing fairly and trying hard, and make my child feel like a winner every time.
    12. I will never ridicule or yell at my child or other participant for making a mistake or losing a competition.
    13. I will emphasize skill development and practices and how they benefit my child over winning. I will also deemphasize games and competition in the lower age groups.
    14. I will promote the emotional and physical well being of the athletes ahead of the desire I may have for my child to win.
    15. I will respect the officials and their authority during games and will never question, discuss, or confront coaches at the game field, and will take time to speak with coaches at an agreed upon time and place.
    16. I will demand a sports environment for my child that is free from drugs, tobacco, and alcohol and I will refrain from their use at all sports events.
    17. I will refrain from coaching my child or other players during games and practices, unless I am one of the official coaches or captains of the team.

    I also agree that if I fail to abide by the aforementioned rules and guidelines, I will be subject to disciplinary action that could include, but is not limited to the following:

    • Verbal warning by official, head coach, and/or head of league organization
    • Written warning
    • Parental game suspension with written documentation of incident kept on file by the organizations involved
    • Game forfeit through the official or coach
    • Parental and/or player season suspension
    ALL RISK WAIVER
    I/We hereby attest that our child/guardian is not exhibiting any symptoms of the coronavirus: mild to severe respiratory illness with fever, cough, difficulty breathing, or other symptoms identified by the CDC. Has not been in contact or exposed to someone with COVID-19 in the last 14 days; has not tested positive in the last 14 days, nor has a medical professional advised me/we to quarantine. 
                       

    I/We, individually, and on behalf of our heirs, successors, assigns and personal representatives, hereby agree to discharge, release, and hold harmless the facility/club hosting the match, their respective Board members, Trustees, officers, employees, agents, independent contractors, or anyone affiliated with the Greenville Junior Team Tennis organization, including its Board of Directors, employees, volunteers, coaches, and captains, from any and all liability whatsoever for any and all damages, losses or injuries (including death) I/we sustain to our person, while participating in today’s match.  
    ​                  

    I/We agree to abide by the amended Rules and Regulations set forth by the club/facility due to the Covid-19 restrictions. Please refer to the facilities postings and signage before entering the courts.

Complete TENNISLINK REGISTRATION. This requires:

Email coordinator@greenvillejtt.com if you don't have a team.


Please be sure to check the "OPT IN" box to ensure delivery of timely information from your Coach, Captain or Coordinator. You will receive an email confirming your registration.
